Please submit a SIGNED and DATED copy of your 2009 Federal Income Tax
1040, 1040A or 1040EZ form, all pages. If you did not keep a copy of
your tax return, your tax preparer should be able to provide a copy or
you may request a copy from the Internal Revenue Service. Request a
free transcript from the IRS by calling 1-800-829-1040 (Form 4506-T) or
printing the form at http://www.irs.gov/pub/irs-pdf/f4506t.pdf. and
submitting to the IRS. Please note: Free transcripts do not include
schedules and may not be sufficient to verify your information if you
are self-employed. If unemployment was received, please include a copy
of your 1099-G.
