Return for 2009 and need more information to complete the verification
process. Please submit a copy of any of these forms that were filed
with the tax return: Schedule C or C-EZ ((Profit or Loss From
Business), Schedule D (Capital Gains and Losses), Schedule E (Income or
Loss from Partnership or S Corp.), or Form 4797 (Sale of Business
Property), Form 8582 (Passive Activity Loss Limitations), or Schedule F
(Farm Income or Loss). If unemployment compensation was received,
please include a copy of your 1099-G.
